Overview
Melvin, a 50-year-old African-American male, is a senior accountant at Procter & Gamble’s headquarters in Cincinnati, Ohio. Melvin works 50 hours per week or more and devotes the rest of his time to his family responsibilities. He has been married for 24 years to Theresa, a sales clerk at a local department store. They are the parents of five children who range in age from 6 to 20 years and are students at the local elementary school, high school, and community college.


Background
Melvin recently decided to purchase life insurance and was required to undergo a physical examination as a part of the application process. The nurse practitioner, Ms. Riley, is reviewing Melvin’s medical history and the results of his physical before developing a care plan. Melvin’s family has a history of coronary heart disease (CHD); both his father and his paternal grandfather suffered heart attacks before age 60. He has had no surgeries, he has not been diagnosed with health conditions, and he takes no medications, yet he complains of fatigue and occasional headaches.
